Partridge Care Centre is Rushcliffe Care Group's newest flagship Care Centre located in Harlow, Essex. Kingfisher Suite Our Kingfisher Suite specialises in Care of the Elderly, including those individuals with Dementia. We aim to deliver the highest possible standard of Person Centred Care, valuing our Residents' as unique individuals. Prior to admission each potential Resident will have a thorough assessment of their needs, and this will be our opportunity to give information about the Home, and ensure that the service we offer is suitable for that individual. All potential Residents' and their family or friends, are welcome to visit the Home, (and spend time with other Residents' and staff) to help them decide if the Home is the right one for them. In conjunction with our Residents', we will develop Care Plans. These plans will focus on the individuals personal preferences, and support them to be as independent as possible, within their limitations. We encourage integration within the local community by assisting Residents' to access local facilities, participate in local events, and by inviting local groups into the Home to support us. Residents' are encouraged to take an active part in planning their routines, and social activities within the Home. Routines are flexible, and aim to meet the needs and wishes of the whole Resident group. Care will be supported by the Residents' GP, District Nursing Team, and GP referral to specialist healthcare facilities/professionals, as required. Our staff team include Support Workers, Activity Organiser, Kitchen Staff, Ancillary Staff, Administrators, and Maintenance personnel. All staff receive training relevant to their job role, as well as training specific to the resident group i.e. Understanding Dementia. We aim to provide a clean, safe and comfortable environment which reflects the individuality of the Home, and it's Residents'. In particular we encourage Residents' to personalise their own rooms, with familiar items, making it as homely as possible. Mallard Suite Mallard Suite offers round-the-clock Specialist Nursing Care for adults who require highly specialised neurological, physical disability, Challenging Behaviour and for those presenting with complex nursing needs who may require access to the Multi Disciplinary Team. Our Service Model This highly specialised unit (9 beds) provides placements with tailor-made rehabilitation and long term packages. The team is nurse led with emphasis on measurable outcomes. All packages of care are pre-determined, person specific and have an ongoing assessment/re-assessment process to determine outcomes. The unit has among other facilities:
  • Hydrotherapy pool
  • Gymnasium
  • Full Tracking systems in most rooms for safe handling of clients.
Treatment Philosophy We are committed to:
  • Creating an environment that is conducive to, and enhances the process of rehabilitation or maintenance.
  • Comprehensive and regular planned assessments.
  • Treatment that is focused on reducing the severity of the symptoms of dementia or the frequency of challenging behaviours.
  • Meaningful and personalised activities that maintain and enhance their life skills. Taking into account the interests of our residents through individual and group work; including dementia care mapping, memory clinics and reminiscence therapy.
  • Helping the resident to maintain strong links with family and friends, through regular contact and visits.
  • Regularly reviewing the effectiveness of care through planned case review meetings.
Our Care Team In addition to a full range of ancillary services, Partridge Care Centre has a dedicated multi-disciplinary team consisting of:
  • Registered Manager
  • Deputy Manager
  • Occupational
  • Therapist/aide
  • Team Leaders
  • Registered Nurses (Mental Health)
  • Registered Nurses (General)
  • Healthcare Support Workers (educated to NVQ Level 2 minimum)
  • Advocacy Services (referrer)
  • Dedicated Activity Coordinator
  • The General Practitioner (who will conduct twice weekly home visits to review residents).
In addition to this, our care support team will provide mentorship to nursing, occupational therapy and/or psychology students or Adaptation nurses when accessed. Who can benefit from our service? Persons from the age of 18 upwards in the following registered categories:
  • PD - Physical Disability
  • Neurology Care - Rehabilitation and long term maintenance
  • Traumatic Brain injury (acquired)
  • Continuing care (Neurological Degenerative conditions), and/or end of life
  • Mental Health (specifically with Neurological or cognitive conditions such as A.B.I. or MS)
  • Continuing Care (Palliative; End of life; Gold Standards Framework)
  • A primary presenting diagnosis of challenging/cognitive behaviour and/or complex needs (e.g. psychosis or dementia).
  • A significant degree of underlying or co-existing cognitive impairment (associated with A.B.I.)
  • Men or women who are difficult to be catered for in a more ordinary residential community setting, due to the severity of their challenging behaviours.
